LOGIN INSTRUCTIONS:
     A. Enter your personal identification number in the field marked "Juror ID Number". Your personal identification number will be called "ID Number" on the Candidate Juror Questionnaire.  It is a 9 or 10 digit number located immediately above the bar code at the upper center of the document.
    B. Enter your 5 digit Zip Code.
    C. Enter your date of birth. (MM/DD/YYYY).
    Click the SIGN IN button one time only.
 
If you are unable to fill out the questionnaire on-line, please complete and submit the questionnaire by mail.
